Types of Breast Implants
Saline Breast Implants vs Silicone Breast Implants
Dr. Polavarapu will help you determine the choice of implant filler, implant size, shape, and other features based on your breast anatomy, body type, and desired increase in size. Breast implants are medical devices with a solid silicone rubber shell. The shell may be filled with either saline solution (sterile salt water) or elastic silicone gel. The U.S. Food and Drug Administration (FDA) has approved both saline and silicone gel breast implants. The size of a breast implant is measured in cubic centimeters based on the saline or silicone filler’s volume. Breast implants vary both by filler and in size, but there are additional features to consider:

Adult women of any age can benefit greatly from the enhancement breast implants provide. However, Dr. Polavarapu usually recommends that a woman’s breasts are fully developed prior to the placement of breast implants. Saline implants are FDA-approved for augmentation in women 18 years of age and older. Silicone implants are FDA-approved for augmentation in women age 22 and older. Saline or silicone implants may be recommended at a younger age if used for reconstruction purposes.
You should be aware that breast implants are not guaranteed to last a lifetime, and you may require future surgery to replace one or both implants. Regular examinations for breast health and to evaluate your implants’ condition are essential whether you have chosen saline or silicone breast implants.
Occasionally, breast implants may break or leak. Rupture can occur as a result of injury or even from the normal compression and movement of your breast and implant, causing the artificial shell to leak.
If a saline-filled implant breaks, the implant will deflate in a few hours, and the saline will be absorbed and naturally expelled by the body.
If a silicone-gel-filled implant leaks or breaks, the elastic silicone gel may remain within the implant shell or escape into the breast implant pocket (a capsule of tissue surrounding the implant). A leaking implant filled with silicone gel may not deflate and may not be noticeable except through imaging techniques such as an MRI.
For this reason, we advise women with silicone breast implants to visit Dr. Polavarapu annually to assess that the implants are functioning well. An ultrasound exam or MRI screening can evaluate the condition of breast implants; it’s recommended that all silicone implants be properly screened after three years.
Mammography is technically more difficult following breast implant placement. Obtaining the best possible results requires specialized techniques and additional views. So, be candid about your implants when undergoing any diagnostic breast exam. Dr. Polavarapu may recommend an ultrasound exam or MRI in addition to mammography.
While the majority of women do not experience these complications, you should discuss each of them with Dr. Polavarapu to make sure you understand breast augmentation’s risks and consequences.
Breast Augmentation Incision Locations

Transaxillary Incision

Inframammary (breast fold) incision
The inframammary or breast fold incision is positioned where the breast meets the chest. It was the original approach and is Dr. Polavarapu’s favorite technique. This incision offers excellent control and precision without the added risk of nipple nerve damage.

Fat Transfer to the Breast
After speaking with Dr. Polavarapu, some women will decide to have this procedure done as a natural alternative to implants. With a fat transfer to the breast, also called autologous fat grafting, she will use your own tissues to modestly enhance your breast size. Dr. Polavarapu will remove fat through liposuction from your belly, thighs, or buttocks, purifying it before injecting it into your breasts. The fat grows new blood arteries, becoming part of your breast tissue in the desired size and shape.

Breast augmentation can be performed under general anesthesia, so you’ll sleep through the entire operation. In some circumstances, it would be possible to use local anesthesia combined with a sedative to make you drowsy. Working through the incision, Dr. Polavarapu will lift your breast tissue and skin to create a pocket, either directly behind the breast tissue (submammary or subglandular placement) or beneath the pectoral muscle and on top of the chest wall (submuscular placement). Once she positions the implant within this pocket, the incisions are closed with sutures, skin adhesive, and/or surgical tape. A gauze bandage may be applied over your breasts to help with healing. The surgery usually takes one to two hours to complete. You’ll want to discuss the pros and cons of these alternatives with Dr. Polavarapu before surgery to ensure you fully understand the implications of the procedure she recommends.
After Your Breast Augmentation Surgery
Post-Surgical Expectations
You’ll likely feel tired and sore for a few days following your surgery, but you’ll be up and about in 24 to 48 hours. Dr. Polavarapu will give you post-operative instructions to promote your recovery and lower your risk of complications. She will prescribe medication to control most of your discomfort.
If you have gauze dressings, they will be removed within several days, and you may be given a surgical bra. You should wear it as directed by Dr. Polavarapu. You may also experience a burning sensation in your nipples for about two weeks, but this will subside as the bruising fades.
Your stitches will come out in a week or two, but the swelling in your breasts may take three to five weeks to disappear.
Depending on the level of activity required for your job, you should be able to return to work within a few days.
Follow Dr. Polavarapu’s advice on when to begin exercising and everyday activities. Restrictions on heavy lifting or otherwise using your shoulders tend to be rescinded in four to six weeks. Your breasts will probably be sensitive to direct stimulation for two to three weeks, so you should avoid much physical contact. After that, breast contact is fine once your breasts are no longer sore, usually three to four weeks after breast augmentation surgery.
Your scars will be firm and pink for at least six weeks. Then, they may remain the same size for several months or even appear to widen. After several months, your scars will begin to fade, although they will never disappear completely, but Dr. Polavarapu specializes in making incisions in places where scars are difficult to see after they’ve healed. Your breasts may continue to settle into their improved status over six to twelve months.
For women in the appropriate age group, routine mammograms should be continued after breast augmentation. However, as discussed earlier, the mammography technician should use a special technique to ensure a reliable reading.
For many women, the result of breast augmentation can be satisfying, even exhilarating, as they learn to appreciate their fuller appearance.
Even if you believe your implants are functioning well, you must follow up with Dr. Polavarapu as directed to assess the condition of your breast implants. In addition, whether you choose to have breast implants or not, it is essential to your health that you practice a monthly breast self-exam and schedule regular diagnostic breast screenings.
